Inside a refugee camp in eastern Sudan, a woman stood at a food distribution point and faced a choice that no person in crisis should ever have to make. MSF staff members, she later told investigators, had made clear that access to rations depended on sexual compliance. She was not alone. An internal investigation by Médecins Sans Frontières has uncovered a pattern of sexual abuse and exploitation spanning months, affecting multiple victims who remained silent out of fear that speaking out would cut off their access to aid.

The scandal exposes a brutal paradox at the heart of humanitarian work in conflict zones. The very organisations that exist to protect the vulnerable can, when accountability breaks down, become instruments of exploitation. Victims in the Sudan camps said they chose not to report abuse because they depended entirely on the aid workers for survival—food, water, medical care—and had nowhere else to turn.

MSF's investigation, released this week, found that staff members had used their control over aid distribution to coerce sexual contact. The organisation has suspended operations at affected sites, referred findings to law enforcement, and begun a broader review of safeguarding practices across its Sudan operations. But the damage to trust among refugee populations is already done.
